Many insurances cover acupuncture. Rates and payments depend on each company. To find out if you have acupuncture coverage under your plan, please submit this form. Once you submit your form, it may take up to 2 business days for us to verify your coverage. We will get back to you with a summary of your coverage and benefits. If you wish to come in for your initial appointment before verification, then you will be responsible for full upfront payment. We will reimburse or credit you for any charges covered by your insurance.
Many insurance policies cover acupuncture care but this office makes no representation that yours does. Insurance policies vary greatly of deductible and percentage of coverage for acupuncture care. We require that you, the patient, be personally responsible for the payment of your deductibles and copays, as well as any unpaid balances in this office. We will do our best to verify your insurance coverage, and will bill your insurance in a timely manner.
We are in-network with the following insurance companies:
Cigna
If we are out-of-network with your insurance company, but you DO have acupuncture coverage, we will provide you with a superbill which you can then submit to your insurance for reimbursement. You are responsible for upfront payment at our non-insurance rate.
We also accept automobile insurance medical coverage (Med Pay). Med Pay pays 100% of necessary medical care for injuries sustained in automobile accidents, regardless of fault. Call your carrier to see if this benefit is included in your policy, or let us check for you.
Workers’ Compensation insurance covers 100% of necessary medical treatment for injuries resulting from one’s occupation (up to 24 visits for acupuncture, not including physical medicine). If you have an open Workers’ Comp claim, or believe that your condition arose as the result of your occupation, you may have benefits.
